On this website you design your own label, they have templates you can choose from. they sell the personalised wine from $9.95 per bottle, sold in cases of 12 bottles, with a minimum order of 1 case. It also costs $8 for nation wide shipping per case.

This website has different types of wine you can choose from, even bubbly, port or non-alcoholic. You can provide them with text and image, or get their assistance and images. They design the label and you approve it. They then wrap it in a nice wooden case and deliver it personally. The personalized bottles cost between $24.50 and $60 (including design an packaging, excluding delivery).
